Intake Form Submission

Fill out out the intake form we send you after your initial contact. Tell us a bit about your recovery journey so far and how we can help.

Review & Approval

Our team will review your information and confirm if Serenity at Bethany is the right fit for your recovery journey.

Personal Welcome

Once approved, we’ll schedule a personal welcome meeting to review the house rules and structure, and answer any final questions you may have.

Eligibility & Criteria

Must be in active recovery or have completed a detox program.
Able to follow house rules, including curfews and substance-free environment.
Willingness to engage in group activities and life skills training.
Committed to long-term recovery and following a structured daily routine.

What to Bring

Comfortable Clothing & Shoes – For daily activities and outings.
Personal Hygiene Items – Shampoo, soap, toothbrush, etc.
Prescription Medications – Non-narcotic medications only.
Bedding & Towels – Soft, comfortable linens to make your space feel like home.
Photo ID & Insurance Information – For medical and emergency records.
Notebook & Pen – For journaling or taking notes during group sessions.
